Description/Job Summary

The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness.

The primary role of the Case Manager is to provide support with an emphasis on monitoring for health & safety, assisting with ADL training, coordinating services with external entities for benefits and other community resources for individuals receiving services through the Enhanced Supportive Housing program.

Schedule: Tuesday- Saturday 8AM- 4PM

*****Program Case Load will streatch across the Durham and Raleigh areas of North Carolina ********

Responsibilities/Duties

• Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.


• Assist in the development of recovery plans and reviews in collaboration with the consumer, natural supports and consented stakeholders.


• Provide community-based services to consumer focusing on education, connection with resources, community interaction and achievement of recovery plan goals.


• Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.


• Develop and maintain relationships with community agencies in order to assist person in connecting with all available resources.

Required Experience

2 years' experience working in a residential facility or mental health setting is preferred.

Required Education

Bachelor's degree in Human Services field i.e Social Work, Psychology, Criminal Justice etc.

Required Qualifications

• Valid Driver's license required.


• Must pass criminal background check

RHD North Carolina's Enhanced Supportive Housing Program (ESH) Program provides housing for persons with serious and persistent mental illness. Members of the team work together with the individuals served to promote symptom stability and appropriate use of medication; to restore personal, community living and social skills; promote and maintain physical health; establish access to entitlements, housing, work and social opportunities; and promote and maintain the highest possible level of functioning in the community.
