Job Description:

Assess client's needs using evidence-based validated assessment tools (i.e. ASI, etc.).
Diagnose clients utilizing DSM criteria for SUD and MH disorders, within the scope of practice as defined by licensing regulations.
Determine appropriate level of care based on ASAM criteria utilizing the Level of Care Index (LOCI)
Complete intakes for new admissions in all required systems (EMR, NJSAMS, etc.), and discharge clients from the same as needed or assigned.
Develop a mutually agreed upon treatment plan to address the clients multidimensional needs, and update the plan as determined by DOH regulations. The treatment plan must be client-centered; recovery oriented and includes goals with measurable objectives, and inclusive of family involvement.
Obtain collateral information from prior treatment episodes to support the current episode.
Provide substance abuse counseling services (i.e. individual counseling, group counseling, crisis intervention) utilizing evidenced based skills (i.e. CBT, MI, etc.).
Facilitate groups utilizing evidenced based curricula, with fidelity to the model.
Seek active case consultations and maintain ongoing communication (direct & in-direct) with supervisor, Director, coworkers, and referral sources.
Provide case management services to supplement the client's treatment episode, including recovery supports and for co-occurring disorders.
Engage the client's family (with consent) and referral source to support the client's treatment episode.
Collaborate with nursing and psychiatrist regarding the clients primary care, medication assisted treatment, and psychiatric needs.
Complete weekly billing to submit to supervisor on time.
Participate in weekly Multidisciplinary Treatment Team meetings for assigned clients, clinical supervision, trainings, workshops, and etc. - as assigned by Director.
Keep accurate records and clinical documentation, in accordance with Annex A requirements and organizational policies; complete clinical documentation for all counseling, education services, assessments, reassessments, referrals, and follow-up in electronic record while maintaining quality assurance standards are met.
Document complete and detailed facility incident reports.
Secure continuing care for all clients on caseload, with collaboration from referral source.
Reassess clients need for continuing care utilizing LOCI & update treatment plans as defined by DOH regulations.
