Job Description:

We are seeking a master's-prepared social worker to join our team.

The social worker in this position provides assessment, counseling, and discharge planning for short stay and general pediatric inpatients and their families. Interventions may include but are not limited to the provision of emotional support, referral and linkage to community agencies, coordination of care for medically complex children, procurement of behavioral health care services, facilitation of financial assistance, transportation coordination, family education, etc. This social worker interfaces with insurance providers, community agencies, and members of the multidisciplinary health care team. Provides services in accordance with policies, procedures, and guidelines established within our social work department, the Nemours organization, and regulatory bodies.

Responsibilities:


  • Manage complex assessments of the patient/family's biopsychosocial situation. Establish goals and periodically reassess.
  • Provide crisis intervention services when indicated for the management of domestic violence, psychiatric emergencies, child abuse, and family disputes.
  • Provide therapeutic, goal-directed counseling. Exhibit necessary clinical skills to provide problem-focused, goal-directed intervention.
  • Interface with insurance providers, case managers, and others involved in the authorization of needed patient services. Identify and negotiate differing needs within the family system.
  • Coordinate discharge planning needs of patients and their families in an efficient, effective and timely manner while including the family in the process.
  • Document social work notes in the patient medical record within the guidelines of the social work department policy.
  • Keep apprised of community resources to facilitate provision of optimal assistance to patients and families.
  • Maintain a professional manner at all times.

Requirements:

  • Master's degree in social work from a program accredited by the Council on Social Work Education (CSWE)
  • LMSW/LCSW licensure in the State of Delaware preferred
  • 6+ months experience
