Job Summary:

The MSW provides direct social work services that are based on the treatment plan's established priorities through analysis of psychosocial assessment, counseling and community service coordination. Promotes a supportive, team-work oriented environment. Fulfills departmental responsibilities with the long-range plan, mission, vision and values of Horizon.

Job Duties:

• You will assist with discharge planning to facilitate continuity of care and the transition of patient with personal or environmental difficulties which may interfere with obtaining maximum benefit from medical care.


• You will facilitate interdisciplinary team approach to meeting the broad spectrum of the patient's healthcare needs.


• You will appropriate interpersonal and communication skills.


• You will use analytical and decision-making skills.

Qualifications

• Master's degree in Social Work required.


• Two years of experience in mental health field and/or acute care environment required.


• Computer skills required.


• Ability to take evening and weekend call.


• Basic Healthcare Provider (BLS) Certification required within 90 days of hire or promotion.
