Job Summary

Responsible for the accurate and timely collection and completion of patient information to support the clinical care team and patient care. Responsible to collaborate, communicate and coordinate patient information and documentation with all disciplines within the Home Care Department and clinical providers as required to ensure continuity of care and regulatory compliance. Responsible to provide functions to support the clinical team and clinical service operations in an effort to enhance patient care and safety, meet regulatory guidelines and to facilitate the ideal patient experience. Responsible to verify patient visit data through EPIC scheduling to complete timely and accurate patient billing. Interacts with hospital and home care staff to assist in the discharge of patients and provide continuum of care from the hospital or outpatient setting to home.

Job Responsibilities

1. Provides support to the clinical operational team to meet patient care needs and services.


2. Retrieval of clinical patient information, lab results and follow up appointment documentation to support patient care functions.


3. Responsible for the physician order tracking process for home care patients as directed by the clinical team to meet regulatory requirements


4. Performs clerical functions to assist with the management of the medical record (i.e. scanning, filing etc).


5. Performs patient satisfaction surveys to home care patients per procedure.


6. Assists in performing audits of medical records to support compliance with DOH, CMS, TJC, etc


7. Performs registration functions in clinical systems as appropriate.


8. Schedules patient visits for Home Health, Home Infusion and Home Medical Equipment & Respiratory Therapy Services according to pre-established guidelines and procedures.


9. Schedules Home Care clinicians efficiently to increase staff productivity while following competency requirements based on licensure and documented employee specific competency information


10. Supports clinician paperwork tracking and billing functions as required.

11. Performs other duties and responsibilities as assigned by management.

Required Education and Experience

• High School Graduate and/or equivalent GED


• Two (2) years minimum related experience.

Preferred Education, Experience & Cert/Lic

• Strong interpersonal skills
