A reputable 300+ bed acute care hospital in Maryland is actively seeking an INTERIM Case Manager Maternal Child Health who could begin as soon as possible. 

The INTERIM Case Manager Maternal Child Health will be offered a 13-week minimum contract with local housing support. The Interim Case Manager Maternal Child Services will work a Monday through Friday schedule and will not be required to work weekends or holidays. 

REQUIREMENTS

• Either a Nurse Case Manager (ideally CCRM certification) experience  OR LCSW / LGSW / LCSW-C


• Previous Case Management experience in Maternal Child Health, Obstetrics, or Pediatric Services

This hospital is known for their large Women and Children Service Line, including their Labor and Delivery unit, Postpartum Mother Baby unit, their Level III Neonatal Intensive Care Unit, outpatient OB/GYN clinics, and Parent Education programs. The Mother Baby Postpartum unit offers private rooms that are designed to create a calming, inviting environment for new mothers and their newborns. New mothers have the option to “room-in” with their baby or utilize the newborn nursery. This state-of-the-art unit is fully equipped with the most advanced technology and a postpartum team of registered nurses, nursing support technicians, clinical nursing students, volunteers, and social workers. The department also offers 24/7 accessibility to on-call Obstetricians, Physician Assistants, Nurse Practitioners, Midwives, Neonatologists and other specialists for the postpartum patients or nursing staff to consult. 

The INTERIM Case Manager will: 

• Report to the Director of Care Management and Continuing Care Services


• Provide psycho-social services including: psychosocial assessment, complex discharge planning, coordination of services, resource referral, support group facilitation and consultation to the Maternal Child Health areas of the hospital


• Participates in interdisciplinary collaboration with RN Care Managers, Physicians, Residents, and staff on various units


• Collaborates with service line team members to distribute patient case load
