Provide school-based services as needed, including attending IEPs and conducting individual or group rehabilitative services to consumers open to Children's Services. Meet with and provide case management and rehabilitative services to consumers and their families. Provide linkage to resources and education to support whole-person care. Contribute to multi-agency consultation in the clinic, at schools, Probation, Child Welfare Services, or other agencies involved in consumer's treatment. Provide services in suburban and rural locations throughout Kings County, including home-based, satellite clinics, schools, and other community locations.
