Infusion Services Nurse Practitioner at Huggins Hospital

Job Description:

Under the administrative supervision of the Director of Pharmacy, works collaboratively with Physicians, Advanced Practice Professionals (APPs) and RNs to perform diversified clinical duties which support the infusion center. Treats infusion center patients in a courteous, respectful, and caring manner.
This is a part-time position working 20 hours per week.
Responsibilities
A. Infusion Services
• Visits all patients with new and annual renewal orders during the patient's first visit prior to the administration of the infusion, injection, transfusion, VAD maintenance, therapeutic phlebotomy, or IV hydration.
• Documents encounters in accordance with EMR Best Practice workflows and aligned with the 340B requirements. Ensures rewritten and original orders are saved in the EMR in accordance with 340B requirements.
• Takes responsibility for the patient's treatment by reviewing documentation by the external/initial Provider; documenting agreement with the treatment plan; re-writing the order; documenting the diagnosis; updating the medication list to include the infusion medication; and sees the patient in the office at least once yearly or as needed for infusion order changes.
• Works collaboratively with the Infusion Services Medical Director and other stakeholders to grow and support the infusion services program.
B. Direct Patient Care
• Conducts high quality physical examinations that include thorough medical history taking, diagnostic testing, and appropriate therapeutic procedures within a primary care setting in a professional manner that aligns with established standards of care and practice.
• Formulates an appropriate plan of care based on physical exam, patient history and clinical findings, and collaborates with internal and external providers and specialists as necessary.
• Communicates pertinent follow up information to patients according to established standards of care.
• Recognizes and reports relevant information that requires urgent attention, and ensures follow through.
• Implements orders including diagnostic tests, referrals, medications and follow up appointment needs.
• Answers patient questions and clarifies concerns regarding medications, diet, activity, and health status.
• Provides and promotes effective and compassionate care for patients and their families.
C. Care Coordination
• Refers to Infusion Coordinator as appropriate.
• Works collaboratively and cooperatively with all members of the multi-disciplinary team.
• After appropriate diagnoses, arranges for stabilizing treatment and refers as appropriate to specialty Physicians; facilities, agencies, or resources in the community.
• Communicates with referring specialists and continues to foster ongoing dialogue that ensures follow through for a therapeutic care plan.
• Coordinates treatment with patient and family members.
• Collaborates with Infusion Services Coordinator and Revenue Cycle for Insurance Approvals
a. Medical Necessity
b. Prior authorizations (PA's) and appeals of denials
c. Doc to Doc consultation and reviews
D. Organizational Responsibilities
• Participates in staff meetings and cooperates with Physicians and coworkers to achieve departmental objectives. Reports concerns and problems as they arise.
• Ensures confidentiality of employee, legal, client/patient, budget and all organizational matters.
• Keeps knowledge base current by attending educational programs.
• Supports EMR projects and training
• Reviews/creates orders and protocols as necessary
E. Record Utilization
• Utilizes the current Electronic Medical Record to complete the documented Best Practice Workflows accurately.
• Documents physical findings and patient response on appropriate worksheets and medical record.

Qualifications
Education: Graduate of an accredited Nurse Practitioner program.

Experience: Three years or more as a Nurse Practitioner preferred. Infusion or Oncology experience preferred.

Licensure: Licensed by the State of NH as an APRN with an active unrestricted DEA Registration. Certified by the ANCC. ACLS (Advanced Cardiovascular Life Support) and BLS (Basic Life Support) certifications preferred. Infusion and/or Oncology Certification Preferred.
