This position will report to the TCM Supervisor and provide targeted case management (TCM) services that promote hope and recovery by using strength-based person-centered practices. TCMs meet with and engage clients at intake, assist in identifying client needs and supports, and developing a personalized plan of care. TCMs ensure coordination among services and help clients link and connect to other health and community resources.
RESPONSIBILITIES
act as advocate for persons served;
link persons served with needed services;
monitor persons served progress and provide individualized supports.
Targeted Case Management services and activities should be provided in community locations which may include: persons served residences, medical and behavioral health sites, hospitals, community resource sites, shelters, and jails.
Complete intakes, assessments, service plans.
Provides resources, home visits, referrals, education, prevention and intervention services.
Assures adequate and complete documentation of service plans, progress notes, billing, and all other required documentation for assigned persons served.
Ensures follow-up of all persons served. Maintain expected level of productivity.
Performs other duties as assigned.
REQUIREMENTS
Bachelor’s degree and 1 year of Case Management experience.
Must have knowledge available resources in the service area for people with serious emotional disturbances.
