Work Schedule:

Salaried Hospice/Home Health LPN After Hours, works Week On/Week Off, will be doing in person hospice visits as needed after 5:00 PM to 8:00 AM, Monday thru Thursday, Friday at 5:00 PM and all Day Saturday and Sunday. Next Week off.

Overview:

Under the Supervision of a Registered Nurse or Nurse Practitioner, the on-call LPN triages after-hours and weekend patient needs by phone and makes patient visits as needed. The nurse organizes and home health/hospice care and is experienced in nursing with emphasis on education and communication. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities as necessary.

Responsibilities


    Complete an evaluation and / or a complete initial assessment of patient / family / caregiver to determine hospice needs
  • Communicate effectively with the IDT regarding changes in the patients' condition and plan of care

  • Document all phone calls, teaching, patient assessments, and interventions with quantitative data

  • Take professional responsibility for following policies and procedures and current Medicare regulations

  • Provide professional nursing care by utilizing all elements of the nursing process

  • Assess and evaluate patient's status by initiating and documenting a plan of care, regularly re-evaluating patient / family / caregiver needs, and revising the plan of care as necessary.

  • Initiate appropriate preventive, palliative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by a physician. Evaluates patient's response to treatments / medications

  • Initiate and update the plans of care for hospice patients in collaboration with the Inter-Disciplinary Team (IDT)

  • Facilitate patient's activities of daily living and efforts toward self-sufficiency, quality of life and optimal comfort care. Provides and maintains a safe environment for the patient

  • Ensure that DME / medical supplies and other necessary items and services are available

  • Assist the patient / family / caregiver and IDT members in providing continuity of care

  • Provides hospice services directly to patients in the patient's home or other setting, including but not limited to a nursing facility, assisted living facility, residential care facility, or adult foster home, which is considered to be the patient's home

  • Facilitate professional communications and works effectively between Company employees / volunteers, patients / families / caregivers, physicians, vendors, and other departments and professionals as appropriate

  • Complete, maintain and submit / synchronize accurate and relevant clinical notes and electronic documents regarding patient's condition and care given.

  • Record pain / symptom management changes / outcomes as appropriate

  • Communicate with the physician regarding the patient's needs and reports changes in the patient's condition; obtains / receives physicians' orders as required

  • Teach the patient / family / caregiver self-care techniques as appropriate.

  • Recognize and utilize opportunities for health counseling with patients / families / caregivers to meet plan of care goals

  • Represent Company with integrity and professionalism to all internal and external customers

  • Ensure strict adherence to Company safety and infection control practices

  • Report all incidents and accidents

  • Participate in IDG meetings, staff meetings, department meetings, in-services, QAPI activities, Safety Committee and other related activities

  • Assume responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes

  • Support mission, values and goals of Signature Hospice, Home Health & Home Care

Qualifications/Skillsets


  • Degree in nursing (LPN) from an accredited school

  • Current unencumbered LPN license in office location state

  • Current CPR certification

  • Ability to supervisor hospice CNA/HHA

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

  • Able to perform the Essential Job Functions / Responsibilities of this job, with or without reasonable accommodations.

  • Possess basic computer skills and knowledge of office technology - e.g. email and internet services, facsimile machines, telecommunication devices, etc.
