Job Description:

Nystrom & Associates, Ltd. is a team of mental health professionals that specialize in psychiatric evaluations, clinical social work, and family therapy. We are committed to helping patients in our community who are experiencing personal, emotional, marriage, family or psychological problems. Founded in 1991, we offer an intensive outpatient chemical dependency program, psychological testing, and behavioral health and medication management services.

We are currently seeking a Full-Time Patient Care Coordinator for our team at our Maplewood clinic! Duties include:
Job Duties:

Act as point of contact for scheduling all live intra-office referrals and new patients within the clinic
Register and integrate new patients in emerging programs
Educate patients and clinicians on all services offered within the clinic, including other NAL programs and locations
Follow up with clinicians regarding patient care issues and patient scheduling (i.e. patient needs a referral due to insurance change, patient was hostile with staff, etc.)

Inform OM and AOM when such issues arise.

Coordinate patient services efficiently and assist in referring between all NAL programs
Attend staff meetings and trainings as needed to educate staff on the live referral process and other Registration processes
Maintain awareness of trends, patterns, and need for new services; report these to Office Manager, Administrative Training & Quality Coordinators, Operations Manager, Vice President of Operations, and CEO
Record all incoming calls in accordance with current policy and procedure
Record daily check outs for all programs and send report daily to administrative stake holders
All duties outlined in the Support Staff in addition to Intake and Scheduling Coordinator position description
Attend monthly after-hours mandatory trainings
Assist with shift coverage in the event of staff shortage
Other duties as assigned
Commit to position for minimum of 1 year

Qualifications:

Prior experience working in health care facilities.
Ability to multi-task and handle stress well.
Positive attitude and ability to be flexible.
Must be proficient in use of computers.
Keep all business information strictly confidential.

Hours (Please ensure that you can work the listed shift below):

Monday - Thursday 8:30AM - 5:30PM and Friday 7:30AM - 1:30PM
