Description of Responsibilities
The Social Worker is responsible and accountable for evaluation, coordination and administration of social work services for patients and their families.

Location Director
FLSA Status: Non-Exempt

Responsibilities
Provide social work assessment and services as prescribed by the physician.
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
Provide a written evaluation defining social work service needs, goals, and expected outcomes in conjunction with Plan of Treatment of referring physician.
Participate in the development of overall Plan of Care.
Assist physician(s) and interdisciplinary team members in understanding the significant social and emotional factors related to patient health problems, family response, and care at home.
Maintain patient confidentiality.
Demonstrate an understanding of patient's medical status and updates self on patient/family status as needed or requested.
Maintain effective communication between staff, health care team members, and family.
Assist patients/families in utilizing community resources that will promote optimum financial and emotional support.
Provide physician(s) and PSA with written updates on patient/family needs, progress, goals as requested by physician/health care team.
Other duties as assigned by supervisor.

Minimum Qualifications
Current state license as required by state law and regulation.
Minimum of at least one (1) year medical social work experience -- pediatric preferred.

Physical Requirements
Frequent walking, climbing stairs.
Getting into and out of a car several times per shift when providing visit care, conducting supervisory visits or traveling with a patient.
Repeated or prolonged standing, bending, stooping, kneeling, or crouching.
Occasional rapid movement and physical agility in response to the spontaneity of children and those patients with neurological impairment.
Frequent lifting, carrying, pushing, or pulling, up to 25 lbs.
Occasional lifting, carrying, pushing, or pulling up to 50 lbs.
Transferring, or repositioning patients, alone or with assistance, for feeding, dressing, bathing, or rendering treatment.
Must be able to see and hear to monitor lights, sounds and alarms on equipment.
Must have sufficient strength and manual dexterity to safely operate equipment, including grasping and manipulating small objects.
Must be able to discern changes in temperature or texture by touch.
May be required to provide care while riding in a vehicle.
Must be able to respond to emergency situations in the home or during transport. This may require physical strength, agility, and ability to function under stressful circumstances to evacuate a patient, to provide physical support to the suddenly weak or disabled patient, or to perform one-man CPR for a prolonged period.
Must be able to function in a wide variety of environments which may involve exposure to communicable disease, household animals, latex, or other sources of allergic response and that cannot always be controlled by the employer.
Must be able to wear a TB respirator and other personal protective equipment as needed.
