We are currently seeking Care Workers to serve members in both Durham and Wake county. This position will allow the successful candidates(s) to work a schedule which will include both onsite (in the county served) as well as remote work certain days of the week as approved by their supervisor.
Care Workers support Alliance Complete Care by ensuring that members are linked with and engaged in recommended services and supports. Care Workers assist members with various interventions under the care plan by removing administrative barriers to care, monitoring authorized services, and helping members transition to community-based settings.
Responsibilities & Duties
Complete assessments and screenings
Complete all assessments to identify whether member is receiving the services which will best match their identified needs
Complete required Screening Tools
Retrieve and review historical data to better-understand member’s treatment history
Collaboration with the Alliance Care Team and Provider Networks
Attend all required meetings related to care planning and provider engagement
Notify Care Manager of any new service needs identified during service monitoring
Work with the Provider Network department to provide technical assistance or to discuss concerns to resolve identified issues
Engage with Providers
Identify barriers to service delivery at the member level and work toward individualized resolution with both the member and provider
Ensure that assessments, person-centered plans, discharge plans, and crisis plans are completed and shared with providers with whom the individuals are linked
Report changes in member’s health status to authorized providers
Service Monitoring
For Medicaid C services: conduct face to face, field-based observation of the member’s experience with service delivery per the frequency and requirements outlined in the Medicaid C waiver and Home and Community-based Services (HCBS) standards
For non-Medicaid C services: complete (a) Provider Engagement Tool to assess provider support needs (to engage member in services) and (b) interventions to resolve administrative barriers to care
Review service utilization and documentation as required by the member’s program enrollment to monitor progress toward individualized goals and fulfillment of the intent of the service authorized
Transition Care (TCLI)
Complete required activities needed to transition the member to a housing placement
Complete the TCL tool and coordinate completion of the Comprehensive Clinical Assessment (CCA)
Complete Documentation
Maintain medical record compliance/quality
Ensure timely documentation of Care Coordination activities as required by department policy and procedures
Compliance with Alliance Policy and Procedure
Adhere to all Alliance Organizational Policies and Procedures and Care Management Desk Procedures
Minimum Education & Experience required-
Bachelor’s degree from an accredited college or university in a Human Services field and two (2) years of experience with the population served.
Or
Bachelor’s degree from an accredited college or university in a non-Human Services field and four (4) years of experience with the population served.
Or
Master’s degree from an accredited college or university in a Human Services field and one (1) year of experience with the population served.
Special Requirements-
Current, valid drivers license
Certification in NACCM, NADD-Specialist, and/or CBIS required within 12 months of hire
Knowledge, Skills, & Abilities
Knowledge of Medicaid basic, enhanced MHSUD, and waiver benefits plans
Knowledge of and skilled in the use of Motivational Interviewing techniques
Person Centered Thinking/planning
High level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance.
Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.)

Strong interpersonal and written/verbal communication skills
