Primary Care Physicians for Community-Based Clinic expansion at Vancouver Clinic

Job Description:

At Vancouver Clinic, caring for people is our first priority and the driving force behind everything we do. New providers will find a collegial, supportive, and caring environment of healthcare professionals committed to our patients and community. We are rooted in Southwest Washington and are expanding our services to the Portland metro region.

We're looking for two (2) full-time BE/BC Family Medicine, Internal Medicine or Geriatrics trained physicians for our new Adult Medicine clinic located in Gresham, Oregon. This clinic is in partnership with Humana. The ideal candidate will have a passion for working with seniors, dual-eligibles and medically-complex patients. Language proficiency in Spanish, Russian or Vietnamese is preferred but not required.

The Humana partnership sites represent a value-based delivery model for Vancouver Clinic. Physicians lead a care team that provides multi-layered support to at-risk and medically underserved adults, 18-years and older. The care team includes a Licensed Clinical Social Worker and nursing support.
