PERINATAL CARE COORDINATOR

JOB SUMMARY:

The Perinatal Care Coordinator will coordinate all perinatal activities within Amoskeag Health in collaboration with the Certified Nurse Midwife and Prenatal team. In addition, the role will oversee clinical tracking of CDF reports and all clinical indicators for grant reporting.

JOB RELATIONSHIPS:

Responsible to the Clinical Manager

RESPONSIBILITIES AND AUTHORITIES:

Responsible for the coordination of the Perinatal Program to ensure that perinatal care is provided to all Amoskeag Health clients that are pregnant.
Develop linkages with medical and community agencies to receive and provide client referrals.
Assess patient's need for treatment or advice via telephone call or personal encounter (walk-in). Consults with provider whenever necessary in order to advise patient of most appropriate action; reinforce provider plan of care.
Determine, based on patient need, if patient needs to be seen by a provider and how quickly. Promote continuity of care within limits of staffing availability. Schedule urgent appointments appropriately.
Respond to patient calls within a timely manner. Document all calls/visits; maintain accurate notes on telephone message and attach to progress note in patient's chart. All documentation in a patient's medical record must be complete. If care is not documented it is not valid.
Maintains a procedure for perinatal referrals. Monthly review of prenatal charts for compliance and continuity of care. Case management review of charts monthly.
Tracks perinatal information on a monthly basis. Information to include total number of prenatal clients - number kept inhouse and number referred out for care.
Assist in the implementation of a MIS tracking system for CDF's, OB referrals and case management of all prenatal clients. Create a recall system via mailings and telephone reminders.
Assist in the maintenance of prenatal health education materials for Amoskeag Health.
Maximizes relationships with referral agencies.
Maintain accurate, timely documentation in the client's medical record of client contacts, case planning, and client's plan of care. All documentation in a client's record to be legible and easily understood by anyone who may need to access the medical record.
Provide clear written and oral communication to both Amoskeag Health clients and employees.
Attend and participate in meeting requirements of Amoskeag Health funding sources. This may include, but is not limited to assisting with development, implementation and monitoring of grant proposal. The clinical tracking for grants will be overseen, in part, by this role.
Attend and participate in other Amoskeag Health meetings as directed. If absent, responsible for reading of minutes and signing the documentation.
Attend mandatory safety-related and infection control in-services as designated by Senior Management.
Maintain licensure for current RN status by ongoing professional education.
Sensitivity to cultural diversity of population being served. Maintain client age-related competence.
Other responsibilities as assigned by supervisor.
Assist the second Perinatal Nurse Coordinator to ensure that comprehensive, high quality prenatal care is delivered to all pregnant clients- including, but not limited to: scheduling appointments, pregnancy tests, prenatal intakes, triaging, case management, NSTs, injections
