Job Role Summary
TheTeam Captain RN and Triage Nurse RN are positionsthat allow for a collaborative approach to the patient's healthcare in a multitude of ways.
Team Captain RN- This role is for the nurse responsible for the overall care of patients assigned to their team's providers. This includes risk assessment and coordination of care for high risk patients, tracking quality metrics, delivering elements of patient education, and serving as the principal point of contact for patients and other care providers outside of face-to-face appointments. The Team Captain supervises and directs the activities of team nurses and support staff.
Triage Nurse RN-Triage is an interactive process between the nurse and patient that occurs over the telephone or face-to-face and involves identifying the nature and urgency of client healthcare needs and determining the appropriate disposition. The role of the triage nurse is to assess the patient, provide patient education, and crisis intervention. The goal of telephone triage services is to direct the caller to the appropriate level of care or service in a safe and timely manner in combination with providing self-care advice and direction. The triage nurse aids in directing the patient to the right level of care with the right provider in the right place at the right time.
Blackburn Site
Located at 2700 Dr. Martin Luther King (MLK) Jr. St., Indianapolis, IN 46208, the Blackburn location providesPediatric and Adult Primary Care services. Other service offerings include Dietitian Services, Diabetes Education, Healthy Me, Mental Health,OB/GYN,Financial Counseling, Podiatry,Social Services, and Substance Use Disorder Counseling and Treatment.
Cottage Corner
Located just southeast of downtown at 1434 Shelby Street, Cottage Corner provides Pediatric, Adolescent, and Adult Primary Care services. Other services offered are Dietitian Services, Dental, Diabetes Education, and Healthy Me.
Outpatient Care Center
Located at 720 Eskenazi Avenue, the Sandra Eskenazi Outpatient Care Center offers Pediatric and Adult Primary Care services. Other service offerings include Dietitian Services, Diabetes Education, Healthy Me, Mental Health, and OB/GYN.
Essential Functions and Responsibilities
Leadership:
Innovates, takes risks and experiments to improve outcomes
Manages timelines/deadlines of team projects and site projects as assigned.
Signs off on staff competencies.
Participates in interview process and evaluations for clinical positions.

An active listener to the patient, family, and staff member in conflict.

Financial Accountability/Stewardship:
Promotes practice growth through attracting and retaining new patients
Ensures requests for medication refills, test results, and other patient concerns are completed in a timely manner.
Works patient panels to meet quality metrics by outreaching to patients who need disease- or age-specific screening and testing.
Works with team providers and team members to provide patient education regarding diagnoses, medications, and wellness and self-management skills.
Provides nurse visits for patients assigned to the team, and ensures that team members provide care within their scope of practice.
Initiative & Growth:
Demonstrates a self-directed work effort aligned with team goals.
Works collaboratively in the care and care management of patients and their families.
Technical Skill:
Demonstrates clinical nursing competence, understanding of job expectations and compliance with organizational policies and procedures.

Uses critical thinking skills in the care of the patients/families and to resolve issues within the team.
Job Requirements
Registered Nurse required; with current license to practice in the State of Indiana
LPN to RN -2 years LPN experience required
RN -1 year RN experience required.
One year previous leadership experience strongly preferred
BSN preferred
