Job Description:

The role of Project Specialist manages administrative activities and special projects that support in maintaining the CCP's Central Management Team's workflows with key stakeholders including ACO Partners (administrators of MassHealth) and Community Partner Care Teams across our six Member Organizations, including Bay Cove, Vinfen and JRI. The role partners with CCP's Director of Program Services on administrative projects as well as those related to Care Team training, project facilitation, pilot programs with our ACO Partners, care planning documentation, and data entry.This is a non-exempt position.


The essential job duties/responsibilities of the position include but are not limited to:


•Manage administrative tasks including meeting facilitation, scheduling, and reporting.


•Manage special projects including TA-funded projects and ACO pilots.


•Maintain updated report of outstanding care team documentation.


•Maintain directory of incident reports pertaining to member care for quality review.


•Manage information flow and support communication flow for CCP internal stakeholders.


•Support collaborations with ACO points of contact and community based medical, specialty, behavioral health, and social services providers.


•Provide ongoing support to Care Teams as related to information flow and workflows, particularly as related to care management technology and systems.


•Participate in regular meetings, including weekly Program Services meeting.

•Knowledge of business communication formats memos,minutes, reports


•Strong capacity to pivot between supporting efforts to managing projects


•Strong knowledge of personal computer applications and equipment:Word, Excel, Outlook


•Comfort navigating and organizing healthcare related data in Excel systems


•Knowledge of record keeping systems


Qualifications:
Minimum two years experience in project coordination/project management required. Minimum two years managing and building excel workbooks and organizational systems.
Experience working in fast paced work environment. Skilled at multitasking and managing multiple priorities.


Education Requirements:


Some college credit coursework is required. Associates or Bachelor's degree preferred.
