Job Description:

ESSENTIAL JOB FUNCTIONS
Assist in the preparation of scheduled daily activities appropriate to the needs of the residents that includes, but is not limited to:
Group social activities;Indoor and outdoor activities, which may include daily walks;Activities away from the facility;Holiday events;Spiritual programs and attendance at houses of worship;Opportunity for resident involvement in planning and implementation of the activities program;Creative activities such as arts, crafts, music, drama, and educational programs;Exercise activities;One to one attention;Promotion of facility/community interaction.

Assists in keeping activities starting and ending on timeKeeps accurate reports and notes for every resident interactionReports medical concerns or observations as needed.Keep the activity room clean, in good order and decorated in an inviting manner.Helps decorate for special events.Complete daily documentation of resident participation.
Transport residents from facility to physician appointments, new admit pickups and personal outings.Assist residents in and out of a vehicle.Physically maneuver wheelchair residents up and down a ramp or automatic lift;Secure wheelchair residents in a vehicle.Assist volunteers when appropriate.
Work with families and volunteers to meet resident needs.Encourage resident participation in activities and document outcomes.

Knowledge of:
Safety rules, policies and procedures, infection prevention and control procedures.Facility emergency/disaster response and evacuation procedures.Full understanding of all aspects of residents rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents rights.Abuse reporting and suspicion of crime procedures.Roadways in and around the city.Traffic and highway safety rules and regulations.Precautions necessary to avoid accidents.
Skill in:
Use and care of equipment, tools, and materials used in resident care activities.Working independently to complete daily activities according to work schedule.Organization.Communication both oral and written.Following and maintaining safety standards.Following and effectively communicating verbal and written instructions.Operating a computer utilizing a variety of commonly used and specialized software applications.
Ability to:
Establish and maintain effective working relationships with associate personnel, residents and resident family members.Coordinate and conduct meetings.Set and achieve goals and meet deadlines.Train and motivate others.Read and write clearly.Understand and follow, quickly and accurately, oral and written instructions.Prioritize, organize and utilize good time management.Maintain confidentiality and work under pressure in a dynamic environment with changing demands and priorities.Respond quickly and effectively to many types of situations, including crisis situations.Monitor time and stress levels to ensure effective attention is provided to the residents.Operate vehicle in a safe and economical manner.Abide by traffic rules and sound traffic practices.Operate as an emergency driver in the event of a facility evacuation.Comply with facility policies and procedures.

WORK CONTACTS Regular contacts with associate personnel, residents, resident family members, and visitors.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS
Must be able to lift and bear weight up to 15 pounds daily and between 25 and 50 pounds or more occasionally.Must be able to push and pull 150 pounds or more on a daily basis.Requires constant physical activities including standing, walking, lifting, bending, stooping, pulling, stretching, turning, grasping, squatting, kneeling and reaching.Must be able to wear protective equipment such as gloves, masks, gowns, goggles and gait belts.Exposure to a variety of patient conditions and elements.Visual acuity to determine the accuracy, thoroughness, and quality of work assigned.Visual acuity to gauge the enjoyment of participants at events.Aural acuity to receive detailed information through oral communication, often from persons with speech impairment.Patience and compassion when dealing with others, especially those who are sick, injured, elderly, or from different backgrounds.Flexibility to work when needed, especially evenings, weekends, or early mornings.Frequent interactions with persons who may be verbally abusive to include yelling and cursing.Frequent interaction with persons who are disabled, elderly and emotionally upset and who may be emotional, verbally and physically abusive.Safety-sensitive position pursuant to A.R.S. 23-493.
REQUIRED MINIMUM EDUCATION, EXPERIENCE AND TRAINING
High school diploma, its equivalent or currently obtaining a high school diploma.Obtain / maintain appropriate Arizona Drivers license.
SPECIAL JOB REQUIREMENT(S)
Obtain/maintain Arizona Department of Public Safety Fingerprint Card at employees expense.Provide driving record from Arizona Department of Motor Vehicles at employees expense.Obtain/maintain Food Handlers Card.Obtain/maintain Paid Feeding Assistant course.Obtain/maintain CPR Certification.Complete annual Tuberculosis Testing.Complete in-service training as required.
