JOB DESCRIPTION:


  • Develops and maintains nursing practice standards of care for a specific transplant population.

  • Serves as a liaison and provides consultation and education services to support standards of care. Identifies and resolves clinical issues relating to the transplant patient.

  • Coordinates the collection of clinical data and patient samples for research studies.

  • Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.

  • Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, arranging for long-range beepers, outpatient treatments, and subsequent hospital admissions.

  • Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.

  • Participates in daily patient rounds with transplant team. Participates in collaborative, interdisciplinary research activities related to transplant patients. Performs related responsibilities as required.

    MINIMUM QUALIFICATIONS:



  • A bachelor's degree in nursing. Four years of recent, clinical, medical-surgical nursing experience.

  • Two years in the transplant population preferred.

  • All nurses employed in roles requiring an unencumbered nursing license must have a valid, active license or temporary permit approved by the Georgia Licensing Board.

  • BLS certification required.

    PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.


    ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
