Job Description:

SUMMARY:

This position possesses the clinical expertise for the assessment of patient/client needs for medications and supplies and performs troubleshooting of appropriate equipment and/or services provided to meet those needs. This nurse is skilled in creating care plans for their patients. A nurse should also be skilled in instructing patients and/or caregivers on the care and use of the systems, medications, and supplies they need for their health. Quite often, this care is provided in the home environment, but can in some cases be a person who does extensive training and education in the office.

PRIMARY RESPONSIBILITIES


  • Contact patients to evaluate the utilization of meds and supplies as well as compliance with each.

  • Identify specific supply needs and notifies patients of the time frame for subsequent deliveries.

  • Conduct on-site reconciliations of patient medications with regards to Compliance, Drug Interactions and Patient Health and Safety. Make recommendations to the Pharmacist and/or Attending Physician based on the discoveries made in the reconciliation

  • Handles incoming phone traffic from other health professionals for coordination of care, including orders, lab results, and progress updates.

  • Provides infusions and injections to patients when necessary.

  • Makes sales calls to referral sources.

  • Provides educational and sales presentations to support the sales staff when necessary in coordination with the sales staff.

  • Participates in Performance Improvement functions, primarily through tracking of employee and patient infections.

  • When necessary, provide assistance to others when workloads increase, including the intake staff or infusion.

  • Take initiative in finding ways to contribute to the organization.

  • Be willingly involved in cross-training, when possible.

  • Serve in administrative nursing capacity in compliance with regulatory guidelines and state and federal agencies, as needed.

  • Educate patient and/or caregiver in basic dietary needs, teach and train enteral feed by bolus or mechanical pump.

  • Physical Demands – The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job. · Regularly requires sitting, standing, and/or work at the computer, the phones, and filing and office machines for extended periods of time.

Physical Demands


  • This person needs to have the ability to regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. 

  • Good fine motor dexterity and cognitive ability for effective typing, use of a calculator, writing, and filing.

  • Command of the senses to a level appropriate for the workplace, including vision, hearing, and speech.

Mental Demands


  • This job requires the ability to think quickly in response to both customer requests and customer complaints.

  • Fluency in English (read, write, and speak) is required as the overwhelming majority of customers and contacts communicate only in that (those) languages.

  • This person must be able to maintain sharp focus, be organized, and meet objectives.  · Ability to take readings, calculate figures and amounts, and double-check those figures.


  • Associates or bachelor’s degree in Nursing required.  

  • Clean driving record.

  • Current nursing license 

  • Ability to assist in the sales process in building relationships with referral sources

  • Excellent phone communication skills

  • Self-starter with the ability and desire to makes sales calls

  • Experience in starting and maintaining IV's
