Job Description:

To assist very low-income veteran families who are homeless but have located permanent housing but need assistance moving in or occupying permanent housing but in danger of being evicted. They will assist the veteran to obtain housing stability and enhance their independent living skills by empowering, providing support, and obtaining resources.



Duties include:

* Building a relationship of trust and care with very low-income Veteran families;

* Providing case management to include:

Performing assessment of participant functions and developing/monitoring case plans in coordination with formal assessment of supported services needed, including necessary follow-up activities;

Establishing linkages with appropriate agencies and service providers in the area/community;

Providing referrals and coordinating with VAMCs and other service providers to provide health care services and daily living services to participants;

Deciding how resources are allocated to participants on the basis of need; and Educating participants on issues, such as supportive services availability and participant rights.

* Providing assistance to participants in obtaining VA benefits;

* Providing assistance to participants in obtaining and coordinating other public benefits;

* Providing/referring for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, child care, and housing counseling;

* Completing required documentation (including progress notes) within 48 hours of contact, and entering data into the Homeless Management Information System (HMIS);

* Demonstrating good clinical judgment in decision making regarding participants;

* Demonstrating ability to relate to Veterans and their families in a culturally appropriate manner;

* All other duties as assignedSkills and Knowledge


Job Requirements:

Bachelor's degree in Social Work, Psychology or related field, with minimum of two years experience working with Veterans, homeless population, housing programs. Must have excellent written and verbal communication skills; be organized, able to problem solve.



Skills and Knowledge Required:

Knowledge of the targeted population, experience in advocacy, community education, and commitment to the principles of the project. Knowledge of the Critical Time Intervention (CTI) evidence-based model, excellent verbal and written communication skills, organizational abilities, and creative problem solving skills. Able to coordinate multiple tasks. Knowledge of area community services and resources preferred. Must have reliable transportation, full coverage insurance, and valid TN driver's license.

Must value cultural differences, be able to work as a team member, and be committed to helping Veteran families. Able to create a culture of respect, be flexible, motivate others, and adapt to the participant's pace and needs.
