Nemours is seeking a Home Care Coordinator, FULL-TIME, M-F 8A-4:30P to join our Nemours Children's Hospital team in Orlando, Florida

Job Duties
The Home Care Coordinator (HCC) is an important liaison between the patient/family, provider, and other care team members. The HCC will work closely with the Nurse Navigators to coordinate the arrangement for services that the patient may need after discharge. The HHC will work with insurance companies, Durable Medical Equipment companies, and Home Nursing Agencies


  • Deliver excellent customer service to the patients/families and other care providers. Focusing on family-centered care.
  • Assist the patient/family with selected aspects of treatment plan
  • Locate community resources and facilities for the patient
  • Facilitates patient's continuum of care and transition to the next level
  • Assist in providing patient's with home health agency for IV therapy, wound care, durable medical equipment and other discharge needs
  • Assist with coordination of transfer to other medical facilities
  • Collaborate with health insurance agencies to obtain authorizations according to the patient's insurance policy for any tests, therapies, DME, etc. ordered by the provider.
  • Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's clinical coordinator or Operations Manager.

Job Requirements

  • High school diploma required.
  • Medical Assistant Certification and/or Registration preferred.
  • BLS Certification within three months of employment and continue to maintain as required.
  • Minimum of six (6) months to one (1) year job related experience required; Pediatric experience preferred
  • Customer service or front office experience preferred.
  • Bilingual preferred.
  • Durable Medical Equipment, insurance and home care services experience
