Description:

The PACE Registered Dietitian (RD) is an integral member of the PACE Interdisciplinary Team. (IDT). The RD conducts clinical assessment, develops care plans, and coordinates interventions to support participants’ nutritional health and management of chronic health conditions consistent with PACE policy and procedure and evidenced-based guidelines.

Essential Duties & Responsibilities:


  • Completes nutrition assessments and implements care plans at enrollment, annually, and at status changes appropriately and in a timely manner.

  • RDs oversee nutrition-related competencies for Dietary Assistants in the PACE Center and all PACE Health Aides.

  • Demonstrates the ability to work collaboratively with the participant/caregiver and other members of the IDT in development and implementation of team goals and related plan of care.

  • Refers participants to appropriate in-house and community services, and to other providers, as appropriate (including speech therapy, meal services, SNAP, Wellness Center).

  • Oversees Nutrition Services including PACE Center based meals, home delivered meals, and supper clubs, as well as grocery delivery service and meal preps as needed.

Qualifications and Requirements:


  • At least one year experience with elders.

  • BS or MS preferred.

  • Registered Dietitian (RD/RL) or RD eligible from an accredited Academic institution.

  • Obtains state licensure within 3 months of hire.

  • Maintains State and National registration/licensure.

  • Maintains the Academy of Nutrition and Dietetics Professional Portfolio.

  • ServSafe Safety Certification (including Massachusetts Allergen Training Certification)

  • CDE preferred

  • Spanish-speaking preferred but not required
