Responsibilities/Qualifications/Skillset
The primary purpose of your job position is to assist in the management of clinical staff and proper documentation that supports resident care under the direction of the Executive Director and the Director of Health Services. This coordination shall be in accordance with established clinical standards, the policies, procedures, and practices of this facility, and the requirements of this state.
Responsibilities:
Document information pertaining to changes in residents' functional status, personal needs, appointments, etc. and communicate changes to appropriate parties
Maintain resident records according to the facility's chart order
Accurately measure, record, and assist with the monthly wellness clinic
Assist in the development and data entry of service plan changes and oversee implementation
File resident records and maintain discharge files
Assist in scheduling appointments and coordinating care with outside services
Conduct weekly, monthly, and quarterly audits as outlined in the clinical standards
Ensure proper documentation of all care plans, nurses notes, and all corresponding documentation for the resident records according to regulation
Assist in planning, conducting, and scheduling of in-service training classes, on the job training and orientation programs to assure that current material and programs are continuously provided
Schedule and participate in departmental meetings
Provide orientation and on-the-job training for clinical staff and ensure that their authority, duties, and responsibilities are understood
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents
Assist in the continuous quality improvement committee in developing and implementing appropriate plans of action to correct identified quality deficiencies
Assist residents with activities of daily living in a way that promotes dignity and independence for each resident
Ensure that an adequate number of appropriately trained staff are on duty at all times to meet the needs of the residents. Ensure proper scheduling department working hours, personnel, work assignments, etc. as needed or required
Assist the Director of Health Services in screening, hiring, training, and supervising all care staff. This includes managing performance, recruiting, and all training requirements.
Make recommendations to the Director of Health Services concerning employee's promotion, demotions, transfers, shift changes, salary increases, etc.
Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status
Review and check competence of work force and make necessary adjustments/corrections as instructed by the Director of Health Services and Executive Director
Education & Experience:
Must Possess High School Education or Equivalate
Must have experience as a caregiver and/or medication technician for at least one year. Supervisory experience is preferred.
Resident Care Coordinator experience or similar preferred.
Qualifications:
Must be able to read, write, speak, and understand the English language.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Must be a supportive team member, contribute to and be an example of teamwork and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Must possess the ability and willingness to work harmoniously with other personnel.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations. Must be able to relate information concerning a resident's condition.
Must provide documentation of such certification upon application for the position. Must not pose a direct threat to the health or safety of other individuals in the workplace.
