Description:

Full-time position available performing direct patient care and patient education to patients ages 18 to greater than 100 years in our Adult Medicine Dept. Will also participate in QI activities. The primary care nurse takes a lead role in the prioritization, coordination and delivery of care by the practice team. The primary care nurse provides both direct and indirect care to patients specific to the guidelines/parameters for the department and demonstrates competency for the department specific population age and developmental stage with a focus on social determinants of health, health promotion, and prevention and management of acute and chronic health conditions. The primary care nurse reports to the Administrative Director. Monday - Friday, 8:00 AM – 5:00 PM and will include holiday rotation.

Education

  • Graduate of an Accredited Nursing Program. Licensure in Massachusetts as an RN.  BLS certification Bilingual skills preferred.

Experience

  • 1 year relevant nursing experience in a community health and/or ambulatory care setting preferred.

Skills/Abilities

  • Demonstrated ability to communicate effectively both in writing and verbally. Familiarity with the use of a personal computer in a Windows environment preferred.
