Job Description:

This is a new role to support the mission of CAPP in the southern region and our DCFY contract and is a key member of our interdisciplinary care team. In CHaD, Social Workers help parents to navigate medical systems, identify and reduce barriers to accessing care, provide grief/bereavement support when there are complex medical diagnoses or injuries, assist in connecting to community resources and are an integral part of the patient care team.  In CAPP, Social Workers are present in all clinical visits. In Manchester, we are seeing 6-9 patients/week on average in our CAPP program and a typical appointment can be 2-3 hrs long. Social Workers conduct comprehensive psychosocial assessments, file reports with child protection, assist in safety planning with the family and child protection, coordinate and facilitate MDT meetings to include community agencies and provide brief supportive therapy to parents or children.

Inital training will take place in Lebanon and then once trained your work location would be in Manchester

 

Key Responsibilities:

1. Provides complete psychosocial assessments of patients and families.

2. Provides counseling to individuals and families to help solve social and emotional problems.

3. Coordinates community resources to address/meet identified needs.

4. Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.

5. Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.

6. Organizes support groups.

7. Documents objective, concise and pertinent clinical data in the medical record.

8. Perform other duties as assigned.


 








 


Minimum Qualifications:


  • Master’s Degree in Social Work (MSW) required.


  • Social work experience in an acute, home care, or ambulatory setting preferred.    




  • Strong organizational skills, verbal and written communications skills. 




  • Sound decision making and judgment. 




  • Time management skills. 




  • Negotiation skills. 




  • Proven ability to deal with conflict resolution. 




  • Comfortable with basic computer skills.
