Central Ohio Primary Care Physicians is seeking a highly motivated Registered Nurse (RN) committed to patient center care with strong customer service skills to fill a full time Nurse Triage position in our busy Care Navigation Center located on the Northwest side of Columbus. Hours for this position are 8a-5p Monday-Friday.

The preferred candidate will provide exemplary triage care to COPC's adult patient panel, utilize their clinical decision making skills efficiently, refer the patient on to the next appropriate level of care and possess knowledge of working within an electronic health record system.

Duties/Responsibilities:

• Provide medical triage via telephone to patients.


• Utilize electronic health record and triage systems.


• Document triage episodes for each patient.


• Document phone details in tracking system.


• Schedule appointments.


• Close patient care gaps.


• Ability to offer flexible work hours, including weekends.


• Follow all policies and procedures and perform job responsibilities instructed by direct supervisor.


• Develop effective working relationships with the nurse manager and COPC physicians.

Requirements:

• High School diploma or equivalent.


• Must possess an active nursing license in the State of Ohio.


• Two years minimum RN experience in a hospital and/or medical practice setting preferred.


• Basic knowledge of electronic health record systems.


• Prior phone triage preferred, but not required.


• Ability to offer a flexible work schedule to meet the center's hours of operation.


• Excellent listening, communication, and documentation skills.
