We will reply on you to carry out a comprehensive, individualized program of physical therapy under the supervision of a physical therapist including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals.  Services are an important part of preventive care, rehabilitation, and treatment of patients with chronic conditions, illness, or injuries.

Education

• Graduated of an accredited Physical Therapist Assistant program.

Licensure/Certification

• Relevant experience preferred


• Current state license and/or certification to provide physical therapist assistant services in good standing.


• Valid National Provider Identifier (NPI)


• Valid driver’s license in good standing and access to insured car


• Successful completion of a caregiver background check and pre-employment screen
