Reporting to the Clinical Manager of Social Work, the Inpatient LCSW will work alongside a dedicated team of Inpatient Social Workers and Case Managers. The Inpatient LCSW will serve in a direct patient care role.

The Inpatient LCSW will provide psychosocial assessments, brief patient and/or family counseling, support, resources, and education in support of continuity of care. The Inpatient LCSW promotes patient agency by means of direct patient interaction, consults with providers on case management expertise, and minimizes hospital readmissions.
