In conjunction with SSM Health's System VP Medical Group and Population Health Operations, provides operational support for SSM Health's value-based care strategies, goals and objectives.
Job Description
PRIMARY RESPONSIBLITIES
Works closely with leaders from SSM Health and other key partners to support and participate in the governance of SSM Health population health programs and initiatives.
Collaborates with leaders from SSM Health and other key partners to develop effective population health strategies and prioritize the population health work, including choosing appropriate key performance indicators.
Responsible for clinical operations that maximize SSM performance on value-based purchasing metrics while minimizing financial penalties associated with those same metrics.
Supports the value-based care contracting teams, ensuring opportunities are aligned with success in value-based care and population health management.
Works in partnership with leaders from SSM Health and other key partners to implement Project Vibrance and other population health strategies and initiatives.
Provides ongoing support for the operational aspects of system and regional value-based care initiatives.
Partners with SSM Medical Group leaders to grow and effectively manage SSM attributed value-based lives.
Leads efforts to broaden reach and effectiveness of SSM Health's population health initiatives through development and support of new clinical networks and delivery models.
Ensures that population health strategies and initiatives are aligned with SSM's financial goals.
Provides business and operational support for partnerships and third-party relationships to achieve SSM Health's population health strategies and goals
Works as a liaison with SSM Health IHT and Epic teams in support of Vibrance initiatives.
Collaborates with Clinical Programs to develop and advance population health initiatives.
Works with Post-acute Care leadership to align population health initiatives across the continuum of care.
Performs other duties as assigned.
EDUCATION
Master's degree in business or healthcare related field
EXPERIENCE
Ten years' experience, with five years' in leadership
PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
Frequent keyboard use/data entry.
Occasional bending, stooping, kneeling, squatting, twisting and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
