Position Summary:

Provide extensive outpatient and in-patient education on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.

Responsibilities of this appointment include providing comprehensive counseling in various clinics in Houston staffed by the UT Physicians Women's Clinic. Provide outpatient nutritional education and guidance to advise patients in food, diet and nutrition by developing evidence based medical nutrition therapy treatment plans. Provide extensive outpatient education on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose monitoring.

Position Key Accountabilities:

1. Educate patients on blood glucose monitoring, pattern management of blood glucose, insulin administration and safety guidelines, and insulin pump and continuous glucose monitoring system training.
2. Provide assistance to the patient in obtaining diabetic supplies.
3. Refer patients to community resources.
4. Management of diabetics in the clinic, and via phone.
5. Manage the diabetic program in compliance with the American Diabetic Association guidelines.
6. Provide case management of diabetic patients under physicians' supervision.
7. Provide diabetes training and education to clinical staff.
8. Assist with research projects including data collection, patient recruitment, and other duties as needed.
9. Communicate with patients and families on a daily basis reviewing blood sugar reports.
10. Communicate with schools that have diabetic patients regarding diabetes management.
11. Refill insulin and diabetic supplies.
12. Other duties as assigned.

Certification/Skills:

Registered Nurse (RN) with current licensure or a Registered Dietitian (RD) holding active registration.
Certification as a Diabetes Educator

Minimum Education:

Bachelor's Degree in Nursing or Bachelor's Degree in Food & Nutrition Sciences or related field.

Minimum Experience:

Three years of experience. May substitute required experience with equivalent years of education beyond the minimum education requirement.

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
