Case Manager - Youth Assessment Center at Sierra Vista Child & Family Services

Job Description:

This position provides Early Intervention and Prevention services focused on diverting vulnerable, high-risk youth from the juvenile justice system into community-based, developmental services and supports so that youth break the cycle of delinquent and criminal behavior and activity. The Case Manager ensures that youth coming to the family resource center receive caring and professional case management services. The Case Manager assists youth and their families with implementation of recommended treatment plans so they are able to re-focus their attention into positive and productive behavior and choices. The Case Manager will meet with youth in their home and at their school with a flexible schedule.
Job Duties:
1. Conduct intake assessment with youth, including interviewing parents, guardian and /or other involved adults.
2. Administer Depression screens to referred youth and caregivers.
3. Complete strength based assessment.
4. Evaluate families for crisis risk, educate family members to reduce safety issues in the home and develop safety plans as needed.
5. Provide short-term interventions and ongoing case management services.
6. Engage and support participant's family regularly in program.
7. Develop goals and support youth in meeting identified goals.
8. Network with other youth providers to establish resources.
9. Maintain ongoing caseload with consistent documentation of case management contacts and client progress.

7. Ability to work at various locations, including client homes, schools and community settings.
8. Effectively use de-escalation skills and techniques for crisis prevention.

Responsibilities:
1. Provide access, advocacy, resources and linkage to clients.
2. Interact productively within a multi-disciplinary team and other community organizations.
3. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
4. Attend community events.
5. Report any suspected child abuse immediately to direct supervisor or utilize the chain of command if supervisor is not available.
6. Report any suspected adult/elder abuse immediately to your direct supervisor or utilize the chain of command if supervisor is not available.
7. Report any imminent danger to self or danger to others to direct supervisor or utilize the chain of command if supervisor is not available.
8. Attend staff meetings, designated meetings and trainings.

9. Cross training to support various roles within programs as needed.
10. Make official contacts with schools, counselors, probation officers, attorneys and others connected with the youth.
11. Provide parent education and wellbeing support to families and caregivers of referred youth to help improve family functioning.
12. Conduct presentations and provide outreach throughout Stanislaus County to create awareness of program goals and services.
