RN Care Manager at University of Oklahoma

Job Description:

OU Physicians is Oklahoma's largest physician group and encompasses almost every adult and child specialty.
 
POSITION INFORMATION:  Care management is a collaborative process to optimize patient adherence to the medical plan of care by establishing goals that promote quality outcomes and lower costs of care.  Care managers work with patients to overcome barriers to accessing healthcare, advocate for the patient, and empower the patient through health education.   
 
 

 
PRIMARY RESPONSIBILITIES:  The care manager’s interactions center on primary care office visits and patient-centered treatment plan.  Contact with patients may be by telephone or in-person.  Care manager services include:



  1. Comprehensive patient assessment.  



  2. Develop an individualized patient care plan collaboratively with the care team.  



  3. Medication reconciliation.  



  4. Provide education for patients and caregivers on condition specific self-management action plans.  



  5. Develop longitudinal relationships and monitor patients progress toward goals. 



  6. Conduct transition phone calls post hospital discharge. 



  7. Identify care gaps and assist access to preventative services. 



  8. Performs various duties as needed to successfully fulfill the function of the position.





  1.  

Education:  Bachelor’s Degree in Nursing
 
Experience:  36 months of care coordination and/or experience working in an outpatient ambulatory setting and direct patient care in an outpatient or hospital
 
Certifications/ Licenses:  State of Oklahoma Registered Nurse Licensure, BLS and CPR certification
 
Equivalency/Substitution:  Will accept an Associate’s Degree in lieu of Bachelor’s Degree with 60 months of of care coordination and/or experience working in an outpatient ambulatory setting and direct patient care in an outpatient or hospital experience.
 

Skills:


  • Advanced knowledge of treatment alternatives and health care measures

  • Advanced knowledge of medical terminology

  • Ability to develop and maintain cooperative working relationships with providers, physicians, staff, and other healthcare workers

  • Ability to analyze information and evaluate results to determine the proper course of action while following established standards

  • Ability to prioritize, organize, and complete projects and assignments in a timely manner

  • Ability to practice cultural competence with awareness and respect for patients and family diversity

  • Ability to work independently and as a team member

  • Excellent written and verbal communication skills

  • Must be detail oriented to maintain accurate and timely data exchanges among all entities involved in the patients’ care

  • Must be proficient in computer applications to include MS Office, electronic medical records and/or case management software
