Location: Hagerstown, Maryland
The Youth Outreach Case Manager provides a holistic approach to case management services and providing outreach to program participants who are disconnected youth ages 18-24 experiencing homelessness. This position is responsible for the identification of and outreach to homeless youth in Eastern Panhandle. The Youth Outreach Case Manager will interact with participants, community resources, staff members, volunteers, and other agency representatives. The position also includes coordination of appropriate community services to facilitate client success.
