Responsibilities





Accountable for the evaluation, direction, supervision and provision of medical social services to patients in the home.



  1. Evaluates and assesses patient’s psychosocial status to identify problems that may affect the patient’s health status.

  2. Assists physician and home health care team members in evaluating patient’s social, emotional and economical status, and identify community resources and other possible resources that can aid patient and family in treating and coping with identified problems and issues.

  3. Assesses psychosocial and emotional status and initiates Baker Act if needed, for placement in an impatient psychiatric family.

  4. Makes referrals to Adult Protective Services, as needed.

  5. Sets realistic goals for patient and incorporates the goals in the plan of care.

  6. Provides counseling or intervention to patient according to the plan of care.

  7. Prepares clinical and progress notes to treatment provided to patient and submitted in a timely manner.

  8. Evaluates effectiveness and outcomes of care provided to patient.

  9. Participates in planning for discharge of patient.

  10. Participates in staff meetings and patient case conferences.

  11. Assesses and screens patients that are suitable for placement in a long-term care or assisted living facility when no longer feasible to care for patient at home due to patient’s physical or mental state.

  12. Establishes and maintains effective working relationships with patients from a wide range of socio-economic backgrounds with home health care staff, representatives of other departments and community agencies.

  13. Provides treatment in compliance with the agency policies and procedures and federal and state regulations.

  14. Participates in in-service programs.

  15. Monitors and supervises performance of the Social Worker Assistant.

Qualifications





Education and Experience

Master’s Degree in Social Work or in a human services field required, including but not limited to sociology, special education, gerontology, rehabilitation counseling, and psychology; and previous experience in a healthcare setting working directly with individuals.



Certifications, Licenses, and Other Special Requirements

Must be licensed by the State, if required by the specific State, and an active license in good standing.



Management/Decision Making

Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these.



Knowledge and Skills

Possess extensive knowledge of a distinct skill or function and a thorough understanding of the organization and work environment. Has working knowledge of a functional discipline. Must possess leadership ability and willingness to work effectively with and supervise other associates. Ability to read, write, speak and understand the English language. Ability to make independent decisions. Ability to deal tactfully with physicians, patients, family members, government agencies/personnel and the general public. Must have patience, cheerful disposition and enthusiasm, as well as the ability to handle patients based on whatever maturity level in which they are currently functioning. Ability to seek out new methods and principles and be willing to incorporate them into existing social services. Must be able to communicate information concerning a patient’s condition. Ability to work independently, have flexibility and personal integrity.
