JOB DESCRIPTION:

Facilitates the development, implementation and ongoing adherence to the stroke program protocols and therapies. Ensures the protocols and therapies become the standard of care followed within the facility. Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of stroke. Rounds with physicians on patients with diagnosis of stroke, reviews clinical data. Provides education and clinical expertise to care team, patients and families. Collaborates with physicians and staff in an effort to decrease length of stay. Identifies and integrates patient learning needs, abilities and readiness into the Plan of Care. Works in collaboration with the nursing team to develop and implement individualized teaching plans for patients and families with regards to stroke diagnosis. Collects and analyzes stroke program patient data to assist with documentation, clinical care and improved outcomes. Participates in rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience.

QUALIFICATIONS

* Licensed Registered Nurse in the state of Florida Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions).

* 1-2+ years of related experience, preferably in an acute hospital setting

* BLS Certification

* ACLS Certification

* Proficiency in key data entry, Word, Excel, PowerPoint, and Outlook
