Under the supervision of the Home Health Case Management Supervisor, the RN Home Health Case Manager will be part of a team of friendly case managers responsible for ensuring that patients are admitted and transitioned to the appropriate level of care, have an effective plan of care, and are receiving prescribed treatment. The Registered Nurse Case Manager of Home Health will also be an advocate for services and plans needed during and after patients stay. Ideally, this Case Management Registered Nurse should have between 1 and 3 years of experience, and must be a registered nurse registered.
