Summary:
POSITION SUMMARY The Behavioral Health Care Manager (BHCM) demonstrates excellent customer service performance in that his/her attitude and actions are always consistent with the standards contained in the Vision, Mission and Values of Covenant Health Care and the commitment to Extraordinary Care for Every Generation. The purpose of the Behavioral Health Care Manager is to work with assigned patient population in the primary care setting to promote optimal mental health. In partnership with the primary care practice team, the Behavioral Health Care Manager will offer diagnostic assessment and symptom monitoring and care management surrounding behavioral health. The Behavioral Health Care Manager serves as a resource and coordinates the overall effort of the team to ensures effective communication. The BHCM will support the Primary Care Physician by regularly coordinating with the Psychiatric Consultant and guides families and patients through disease self-management, ensures access to an array of community services, administers behavioral health screenings and conducts and arranges for more detailed assessments when indicated. BHCM assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient s mental health. Delivers brief interventions to support necessary lifestyle and behavioral changes. Responds to crisis situations in the physician office setting. Integrates evidence-based clinical and preventative guidelines, and protocols, in the development of patient centric individualized care plans. Actively manages a caseload of approximately 60 to 120 patients. Provides targeted interventions to avoid hospitalization and emergency room visits. Coordinates care across the continuum and assists patient/families to understand their health care options.

Responsibilities:
Provide screening, diagnostic assessments, and treatment recommendations for common mental health and substance disorders. Therapy will not be provided by the BHCM. Closely coordinate care with the patient s medical provider and Psychiatric Consultant and when appropriate, and, when appropriate, other mental health providers. Addresses and refers for social determinants of care needs, i.e. access to health care, educational, and psychosocial needs of the patient/family. Uses standardized health risk assessment tools. Provide patient education about mental health and substance abuse disorders and available treatments. Based upon the psychosocial assessment and multidisciplinary care planning, provide brief behavioral, crisis, education, interventions to restore the capacity for social functioning. Coordinates patient care through ongoing collaboration with PCP, patient/family, community, and Psychiatric Consultant. Fosters a team approach and includes patient/family as active members of the team. Serves as liaison to acute care hospitals, specialists, and post-acute care services. Responsible for developing a comprehensive individualized plan of care and targeted interventions. Continually monitors patient/family response to plan of care, and revises the care plan as indicated. Facilitate treatment plan changes for patients who are not improving as expected in consultation with Primary Care Physician and Psychiatric Consultant and who may need more intensive or specialized mental health care. Provides patient self-management support with a focus on empowering the patient/family to build capacity for self- care. Support psychotropic medication management as prescribed by medical provider, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload. Participates in regularly scheduled caseload consultation with the Psychiatric Consultant and communicate resulting treatment recommendations to the patient s Primary Care Physician. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case Reviews may be conducted by telephone, video, or in person. Maintains required documentation for all Behavioral Health Care Management activities including patient progress and treatment recommendations in EMR that can be shared with Psychiatric Consultant, other treating providers and codes appropriately for services provided. Track patient follow up and clinical outcomes using a registry. Registry function may be within EMR or spreadsheet. Works with practice and PO/PHO leadership to continuously evaluate process, identify problems, and propose/develop process improvement strategies to enhance Behavioral Health Care Management and Collaborative Care Model. Participate in required continuing education opportunities.

EDUCATION/EXPERIENCE REQUIREMENTS Master s degree in Social Work from an accredited by the Council of Social Work Education, or equivalent degree. Other backgrounds and degrees may include Registered Nurse (BSN), Licensed Psychologist, Licensed Mental Health Counselor/Professional Counselor (LMHC), Nurse Practitioner (NP). Must have current clinical license to practice in the State of Michigan. Minimum three years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting within the past five years preferred. Experience working with patients who have co-occurring mental health, substance abuse, and physical health issues. with behavior health experience preferred. Basic knowledge of psychopharmacology for common mental health disorders. Experience working with community agencies and health care providers. Experience as participant in continuous quality improvement preferred. Experience in self-management support preferred. Excellent computer knowledge and capability to use computer required; i.e. Microsoft Word, Excel. Epic experience preferred but not required. KNOWLEDGE/SKILLS/ABILITIES Demonstrated competence in Social Work or Nursing. Commitment to ethical practice of social work principles (patient self-determination, privacy, dignity, cultural sensitivity) Knowledge of behavioral health conditions, evidence based guidelines, mental health risk assessment, and patient education preferred. Critical thinking skills. Ability to manage complex mental health issues utilizing assessment skills and protocols. Excellent assessment and triage skills. Ability to implement evidence base interventions and protocols for mental health conditions. Ability to travel to work in the physician offices, attend meetings and conferences as needed. Demonstrates customer focused interpersonal skills, and the ability with the understanding of cultural diversity to interact in an effective manner with practitioners, patients, and families. Demonstrates ability to work autonomously and be directly accountable for practice. Demonstrates ability to influence and negotiate individual and group decision-making. Demonstrates ability to function effectively in a fluid, dynamic, and rapidly changing environment. Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization. Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families Demonstrates excellent communication--both verbal and written. Excellent interpersonal and facilitation skills.
