Registered Nurse- $3,000 SOB at Penn-Mar Human Services

Job Description:

Penn Mar is currently looking for a Registered Nurse with two-plus years of experience. The Registered Nurse, will be responsible for the overall nursing care and case management of the individuals residing in Pennsylvania group homes within your caseload and will provide nursing support as needed for the individuals attending the day program.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. 



  • Works with Residential Supervisors, Program Managers and LPN’s to manage the health of individuals supported in the Pennsylvania Residential program.

  • Attend team member meetings and training as assigned by Supervisor or as deemed necessary to fulfill the essential duties of the position.

  • Adjusts scheduled hours as needed when support services are requested. Attend ISP meetings as needed and/or as assigned by the supervisor.

  • Provide emergency nursing assessment to individuals in the Community Living Program and/or PA Day Program.

  •  Attend and/or chair committees within Penn-Mar and outside of the agency as deemed appropriate by the Community Living Administrator.

  • Writes protocols and plans of care, training team members in the implementation of said protocols/plans of care.

  • Complete medical histories within the required timeframe.

  • Complete individual health assessments within the established timeline.

  • Complete monthly reviews of Medication Administration Records.

  • Assumes on-call responsibilities when medical emergencies or hospitalizations occur.

  • During hospitalizations, makes frequent visits, communicates with all health care providers and advocates for the individual.

  • Actively participates in hospital discharges and coordinates team member training for any new procedures.

  • Advocates to community health care providers regarding the appropriate treatment of individuals and the Substitute Decision-Making procedures.

  • Ensure compliance with 6100 and 6400 regulations for applicable licensing and certification.

  • Ensure that the rights of individuals are protected and advocate for individuals as appropriate.

  • Assists in obtaining medical equipment and supplies according to the needs of the individuals.

  • Assures those necessary medical appointments are scheduled.

  • If the appointment is related to a serious health condition, the RN may also be required to attend the appointment to gather essential information from the medical professional to assure proper treatment/care.

  • Completes follow-up paperwork, documentation, and communication with all team members.

  • Assures recommended medical follow-ups such as blood work, x-rays or special studies are secured.

  • Must have a working knowledge of Pennsylvania regulations and nursing state and federal laws.

  • Provides nursing services and direct support as needed to individuals supported by Penn-Mar.

  • Supervise the PA LPN positions in the execution of their duties.

  • Actively participate in designated committees and complete any agency-wide training assigned (i.e. Medication class, BBP, medication refresher class; etc)

  • Other duties assigned.

Requirements:



  • PA RN License. (In good standing)

  • Valid Driver's License with 2 years of driving experience.

  • Human Service and DDA experience preferred.

  • Minimum of two years of experience working in the nursing field. Must have a thorough knowledge of contagious diseases, bloodborne pathogens, medications, and their side effects.

  • Ability to read, analyze, and interpret professional journals, medical reports, nursing and DDA regulations. Ability to write nursing care plan reviews and reports in a clear and proficient manner. Ability to effectively communicate with others especially consumers, family members, staff, service coordinators, and other health providers. Understanding of adult learning and strong teaching skills.

  • First aid and CPR certification.

  • Must pass basic Math and Reading Comprehension Exercise.

  • Able to lift 50 lbs.
