Our St. Francis hospital is seeking a PRN Licensed Master Social Worker. We are looking for your experience, knowledge and patient care skills in our Case Management/Utilization Review Department where you will be responsible for:

What You Will Be Doing:

*

Provide direct discharge planning services and assistance with psychosocial services for the patient and family.
*

Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
*

Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
*

Promote continuity through communication of the patient's plan of care.

So what experience are we looking for in our Licensed Master Social Worker?

Master of Social Work degree from an accredited facility.
*

Clinical emphasis from a CSWE accredited graduate school.
*

State of Kansas licensure at the LMSW level.
*

Prefer two semesters of graduate field experience in healthcare setting for a new graduate OR previous experience in a healthcare setting for an experienced graduate.
*

Broad knowledge of healthcare and community resources.
*

Interpersonally gifted possessing excellent communication and negotiation skills.
*

Self-motivation, tenacious, and enjoy a diverse, fast-paced work environment.
*

Computer literacy.
