On Call Nurse Advice / Anti-Coagulation Nurse at Vancouver Clinic

Job Description:

An exciting opportunity for an on call RN to provide flexible coverage for Nursing Support Services of Nurse Advice and Anti-Coagulation Nurse.

Responsible for providing telephonic clinical assessment and health education according to established protocol. Acts as patient advocate to a broad range of people with varied ethnic, religious, and socio-economic backgrounds. Applies nursing skill to assist patients to maintain health and treat illness. Will enter patient medical and demographic information in an electronic data system, assess caller information, offer clinical advice, and refer callers to appropriate services. Will also educate the client when appropriate regarding immediate care advice and preventive behaviors. Maintain log of calls and action taken and perform quality assurance activities. Models professional behavior through positive interactions with all members of the health care team.

Patient Care


  • Assess callers' needs and assist them with issues related to patient's care.


  • Adapt communication style to persons representing diverse personal, professional, cultural, and socio-economic backgrounds.


  • Ability to multi-task: simultaneously think, talk, and type.


  • Use excellent hearing and listening skills to receive detailed information.


  • Utilize computer database to assess, educate, and document all information in an accurate, detailed manner


  • Triage and prioritize patient needs based on nursing assessment, availability of resources and patient preference.


  • Possess working knowledge of community resources to assist patients in meeting health care needs.


  • Provide and direct patient education, advice, and procedures as appropriate.


  • Assist patient in obtaining necessary appointments and testing to meet his/her needs.


  • Meet minimum established quality and productivity standards.



Communication

  • Process phone calls in a timely manner.


  • Triage voice mail messages to meet needs of acutely ill patients first.


  • Complete Patient Communication Forms (documentation of patient intervention and communication accurately.



Patient Education

  • Educate patients and families in health care issues and needs.


  • Identify educational opportunities for patients and provide information


  • Possess knowledge of opportunities for patient education in the community.



Leadership

  • Acts as a mentor, role model and resources for co-workers.


  • Assess patient needs. If no appointment is available at same site, communicate with nursing staff at other sites until patient needs are met.


  • Work with Patient Services Representatives to assure accuracy of appointment type and billing/insurance information.



Documentation

  • Document relevant information about patients. Ensure entries are objective, factual, time specific, accurate, correctly spelled using generally accepted medical abbreviations and terminology.


  • Ensure entries in chart are appropriately typed, initialed, and dated.


  • Complete Patient Communication form accurately and legibly (documentation of intervention and communication).


Required qualifications for this position include :


  • Current, unencumbered Washington RN license.


  • American Heart Association Basic Life Support (AHA BLS) for Health Care Provider certification at time of hire.


  • Relevant training and certifications applicable to the position.


  • Act as a mentor, role model, and resource for co-workers.


  • Be dedicated to meeting the expectations of patients and co-workers.


  • Interact effectively with patients and physicians in pressure situations while maintaining tact and diplomacy.


  • Work independently while functioning as a team member.


  • Organize and prioritize workflow.
