Overview:

A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The emergency department-based community health worker will be based in a Mercy hospital, working with individual patients as part of an integrated team of healthcare providers.

Qualifications:

* Experience: One or more years of health care or social services experience.

* Required Education: High School Diploma required.

* Preferred Education: CHW certification and Bachelor's degree preferred.

* Other: Strong relationship building and collaboration skills. Excellent communication and facilitation skills. Ability to work independently and function as part of a team, have good time management skills, and attention to detail. Compassion for the poor and underserved members of the community.
