Job Description:

Bachelors degree from a four-year college or university preferred; preferably in psychology, social work, or a related field. Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree.
Summary of Job: The Care Coordinator provides services that help consumers gain access to needed medical, social, educational and other services. This includes direct assistance in gaining access to services, coordination or care, oversight of the entire case, linkage to appropriate services, and ensuring continuation of entitlements.

Provides evidence-based individual and group clinical interventions both in the office and community settings to develop trusting relationships with clients.

Essential Duties and Responsibilities:
Supervisory Responsibilities for: This title does not supervise others.
Facilitate client's use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties.
Assess and develop person-centered plan and monitor progress of the plan.
Clinical documentation is accurate, meets organizations standards in composition and content, and is adequate for communication about client's treatment and progress.
Compliance with Care Plan and ASNA assessments.
Regular and reliable attendance is required.
