Job Description:
The Rehabilitation Specialist will provide assistance to clients in gaining skills that are based on their individualized goals. These skills include, but are not limited to, teaching coping skills to manage/reduce symptoms of serious mental illness.medication management, developing life skills (i.e. cooking, money management, cleaning, parenting, and socializing); pursuing recreational and leisure activities, housing, transportation, legal, medical and financial assistance, and securing appropriate entitlements. The Rehabilitation Specialist will participate in weekly team meetings to coordinate client care and assist in developing person centered care plans.

The Rehabilitation Specialist is also required to meet organization standards, including timely and accurate documentation focused on the progress and treatment of clients. The Rehabilitation Specialist will work with clients in the community or in the office [NOTE: if needed due to current pandemic/office closure, services can be provided remotely via video or similar telehealth platform]. This will be a primarily community-based position.
