SUMMARY

This is a full-time Non-Clinical Case Management position located in Topeka KS. This position provides non-clinical case management services to the CNHS Kansas Army National Guard.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Case Manager Services: Includes the following:


  • 7:00 a.m. and 4:30 p.m. on Monday thru Friday, except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings.
  • Provide Non-Clinical Case Manager Services.
  • The non-clinical case manager requires the expertise as dictated by the "practice of nursing" - the systematic application of substantial specialized knowledge and skills derived from the biological, physical, and behavioral sciences; to the care, treatment, counsel, and health teaching of individuals who are experiencing changes in the normal health processes; or who require assistance in the maintenance of health and the prevention, or management, of illness, injury, or disability.
  • Case Managers/Registered Nurses must have some experience in Behavioral Health and be able to build a rapport with the Soldier and demonstrate empathy to each Soldier's situation. At least one (1) case manager will be responsible for referring a majority of behavioral health cases to the Kansas Director of Psychological and Behavioral Health.
  • The non-clinical direct service case managers require the expertise as dictated by the individual State Licensing boards which set the scope of practice. Case management knowledge domains consist of assessment, planning, implementation, coordination, monitoring and evaluations of the options and services required to meet the KSARNG Soldier's health and human service needs.
  • The case management personnel shall perform the following tasks in the execution of non-clinical case management support:
  • Monitor ALL assigned Soldiers in the eCase module. This includes contacting Soldiers once a month.
  • Monitor eCase to ensure newly assigned Soldiers are identified and contacted in a timely manner
  • Perform medical and administrative charting in eCase & MEDCHART
  • Determine accurate restrictions when generating profiles and track the profile to ensure it is signed by a provider or Deputy State Surgeon in a timely manner.
  • Coordinate Soldier treatment plans with appropriate medical facilities.
  • Maintain automated referrals indicated in MEDCHART.
  • Submit case processing, medical documentation, and profile concerns to
  • military/contacted provider.
  • Attend SRP events as determined by the needs of the state and the individual units.
  • Ensure those Soldiers referred by a provider are followed by the case management team.
  • Manage Soldier's care from the identification of the condition to when the Soldier is either: Considered medically cleared. Administratively separated from the military.
  • Follow the regulations and policies applicable to Individual Medical
  • Readiness (IMR) and deployment, to include the AR 40-501 and other applicable guidance.
  • Ensure all medical documentation received from the Soldier and/or their medical team is received and placed in Soldier's packet, as well as the Soldier's STR.
  • Ensure Medical Record Clerks are provided with all relevant medical/dental documentation for scanning, indexing and filing after closing a case.
  • Obtain release of information from Soldier.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA standards and guidelines.
  • Review and track health assessment (PHA, SRP, CVSP lab results and contact Soldier to educate on lifestyle modifications.
  • Communicate with unit to ensure Soldier attendance to medical appointments and submission of follow up documentation.
  • Log in eCase any contact with SM (email, voice call, or personal interview.
  • Communicate with applicable unit regarding non-compliance, fit-forduty or not fit-for-duty determination for Soldier.
  • Obtain & review requested records from AHLTA (Armed Forces Health Longitudinal Technology Application.
  • Provide a weekly report on number of referrals/cases initiated, active cases and closed cases.
  • Maintain Soldier medical records in accordance with Army Regulation (AR) 40-66

  • Performs other job related duties as required.

SUPERVISORY/MANAGEMENT AUTHORITY

This is a full-time position that reports to the Site Lead and Program Manager.

EDUCATION and/or EXPERIENCE

Active Kansas RN License

Medical Office and/or medical records experience preferred.

Knowledge of military medical records preferred.

High proficiency with learning and using computer programs

Medical and Behavioral Health case management experience preferred

KNOWLEDGE, SKILLS AND ABILITIES

Contractor employee should possess the following knowledge/skills:


  • Strong organizational skills
  • The ability to use Power Point, Excel, Word, Access, Outlook
  • Operate a personal computer to input, store, retrieve and manipulate data for various reports as required by this PRS. Experience with computerized client management systems is preferred.
  • Ability to learn to use various software programs to maintain database files and prepare reports, including but not limited to the following ARNG web-based electronic data systems via MODS: Medically Non-Deployable (MND)/e-Profile, Medical Data Protection System (MEDPROS), MEDPROS Web Data Entry (MWDE), Soldier Patient Locator (SPL), Warrior Transition Report (WTR), DENCLASS, AVS, HRR, ePHA/MHA, eCase and other modules as applicable or as they become available.
  • Experience with the military, such as prior military service or military family member or military family volunteer, is highly desirable.
  • Demonstrates competency in assessing, planning, implementing, and evaluating care provided including patient/family education.
  • Demonstrates leadership qualities including professional verbal and written communication skills, flexibility to prioritize complex situations, and decision-making skills.

WORK ENVIRONMENT

This is a full-time position that assists the different Case Management teams as needed in support of PHA(s) and or Army National Guard soldier medical readiness needs. This position works closely with the Case Managers and other Administrative Specialists to meet the needs of the ARNG CM program.
