Job Description:
Summary of Job:
This position will work in close partnership with the Monroe County Courts, Probation and Jail. Provides evidence-based individual and group clinical interventions in the community settings to develop trusting relationships with clients. Some groups will be facilitated in the jail and will work on transition planning with incarcerated individuals. Engage with clients so that they may more freely discuss substance use and its effects on mental and physical health and daily functioning. Aid clients in setting their own goals for treatment, as well as plans to reduce or abstain from substance use or other maladaptive coping strategies and to learn healthy coping skills. Build rapport and a therapeutic relationship with clients to introduce them to all the elements of a healthy recovery. Assist clients with being linked to community resources and then facilitate communication, care coordination, etc. Coordinate care of clients with the Monroe County Courts, Probation and Jail by having open communication about progress, concerns, crises, and other important information.

Essential Duties and Responsibilities:
Supervisory Responsibilities for: This title does not supervise others.
Facilitates client's use of family, social and community resources; acting as an advocate with external resources and agencies and self-help programs.
Provide individual, group and family counseling covering psycho-educational, addictions, daily living skills and activity therapy.
Develop, implement and monitor clinically and fiscally responsible individualized treatment/discharge plans.
Participate in treatment team meetings for the purpose of summarizing, reviewing and developing client's treatment goals, progress and use of services.
Maintain necessary communications and reporting with referral sources and other involved parties as approved by the client; provides testimony in court in regards to client's behavior and condition when requested.
Regular and reliable attendance is required.
Coordinate care of clients by having open communication about progress, concerns, crises, and other important information.
Assist clients with being linked to community resources and then facilitate communication, care coordination, etc.
Key Performance Indicators
Meet clients as often that meets their level of need.
Complete clinical documentation within one business day of the service provided.
Complete weekly compliance reports for clients involved with the Monroe County Courts, Probation and involved with jail services.
