Responsible for contacting physicians and hospitals to obtain Face to Face document. Requires heavy use of computer equipment. Responsible for accurate maintenance and follow through. Requires excellent organization skills and professional and calm attitude. Reports directly to Personnel Manager.

RESPONSIBILITIES

* 1. Print and track admissions requiring Face to Face form.

* 2. Capture incoming Face to Face documents.

* 3. Evaluate document information for accuracy.

* 4. Follow up on missing or inaccurate information with medical offices.

* 5. Enter documentation into medical record.

* 6. Maintain Clinical Support Fax Mailbox - Forward /Fax/Print/ documents.

* 7. Cover Clinical Support Secretary and Clinical Support Clerks on vacation and when needed.

* 8. Track - refax Medication Reconciliation Forms.

* 9. Provide documentation for ADR?s, attorneys, patient requests.

* 10. Pull discharge records

* 11. Complete discharge process

* 12. Process New admissions chart.

* 13. Models appropriate professional appearance and behavior.

* 14. Projects a positive attitude and enthusiasm about the agency mission, philosophy and policies.

* 15. HIPAA Access Level 4.

QUALIFICATIONS

EDUCATION

High school graduate with office experience including data entry, typing, filing, and word processing. Satisfactory work references.

SPECIAL SKILLS

Light to medium work. Constant handling of up to 10 pounds to occasional handling of 50 pounds. Clear vision required.
