Job Description:

The primary purpose of your job position is to assist in planning, organizing, developing and carrying out individuals receiving services restorative programs, as well as providing direct care on the floor as an NAR as required, in accordance with our established policies and procedures and as may be directed by your supervisors to ensure that the highest standard of services is maintained at all times.

Minimum Requirement: You must be a Nursing Assistant (NAR) on the MN State Registry in good standing or the ability and plan to complete Nursing Assistant course work and/or complete skills and written competency testing in accordance with regulations and care center policy.

Responsibilities:


  • Participate in and receive the nursing report upon reporting to duty.

  • Follow established policies concerning exposure to blood/body fluids.

  • Follow established policies/procedures for performing all cares.

  • Follow written NAR procedure for designated shift.

  • Attend to daily care needs per each resident’s plan of care (e.g. bathing, oral care, hair care, nail care, transfers, feeding, ambulation, R.O.M., elimination needs, intake, output, catheter care, dressing, turning and repositioning, collection of specimens, weights and taking vital signs) and document the care each time it is provided.

  • Observe residents regularly and respond promptly to call lights, while showing respect for resident’s privacy.

  • Report changes in residents’ condition and/or normal routine including nutrition, elimination, ambulation and skin integrity.

  • Turn all medications found in the resident’s room/possession (that are not care planned to be there) over to the Director of Nursing Services or Nurse Manager.

  • Provide considerate and respectful care, preserving the personal dignity, modesty and privacy of the resident and family.

  • Communicate (intra shift) with co-workers, supervisor, and DON as appropriate, regarding resident’s progress and your progress with daily resident’s care.  


  • Individual Responsibilities:



  • Demonstrate the ability to obtain information and data in terms of resident’s needs.

  • Demonstrate an understanding of the range of treatment needed by these residents and ability to provide treatment.

  • Possess communication skills necessary to interpret age specific response to treatment. 

  • Practice nursing policies and procedures concerning resident restraints and side rails. 


  • Specific Requirements:




  • Able to operate the following equipment:

    • Bathing tubs/showers

    • Mechanical lifts

    • Charting

    • Transfer belt



  • Specialized chairs, wheelchairs, and positioning equipment

  • Urinary catheters and ostomy bags

  • Dental Care

  • Hearing aids 


  • Physical and Sensory Demands:



  • Must be able to walk, stand and sit for 4-8 hours in an 8-hour shift.

  • Must be able to lift, carry, or pull 30 pounds.  Rarely an NAR may be subjected to lift more than 30 lbs in an emergency situation.

  • Must be able to push 50 lbs.

  • Must be able to lift 31- 60 pounds with assistance. 

  • Must follow facility policy and procedures when using a mechanic lift is warranted.
