The Life of an HCA Nurse

RESPONSIBILITIES:

The Case Manager coordinates organization efforts to efficiently manage admissions, plan of treatment and discharge needs for patients admitted to the hospital. Reviews admissions for appropriateness, effectiveness of treatment, efficiency of services and timeliness of discharge to ensure all patients receive the highest quality of care/service possible in the most appropriate setting.

QUALIFICATIONS

* Current Louisiana RN License or Compact License; RN Diploma or Bachelor's Degree

* Level 1: 3 years experience in clinical acute care; 1 year case management experience preferred

* Level 2: 3 years experience in hospital case management or 2 years experience in hospital case management with one year experience of other case management
