This individual will join a national healthcare organization, accountable for overseeing all Nursing Services over the facilities Long Term Care, Outpatient Rehabilitation, and dedicated Short Term Care Unit. 

The Long Term Care Nurse Director will join a nationally recognized health system, working at their 175+ bed facility. Services offered at this facility include Long Term Care, Outpatient Rehabilitation, and a dedicated short term care unit. The dedicated short term care unit provides post-hospital care, designed to help patients transition home or to a lower level of care after a hospital stay. This unit utilizes a dedicated, interdisciplinary team of RNs, therapists, dieticians, social workers and staff, all focused on managing discharge planning to minimize the probability of returning to the hospital. Additionally, this facility offers outpatient rehabilitation programs for both their residents as well as other patients recently discharged from their recent hospital visit. This facility is recognized for their Long Term Care Program, offering an array of long term care residential options with varying levels of care. To ensure the safety and exceptional care, residents have 24/7 access to a team of RN’s and staff. 

The facility is dedicated to delivering individualized patient care while creating an “at home”, inviting environment. To create a personalized, at home environment for residents, the facility offers an array of amenities and programs. Administrative responsibilities of the Nurse Director Long Term Care will also include operational work flow across the different services offered, ensuring all federal, state, and department of health standards, guidelines, and regulations are in compliance, updating policy procedures, preparing survey and accreditation material, implementing quality performance improvement initiatives, and assisting with staff hiring, development, education, and evaluations.
